Any personal
data you provide will be held securely and in accordance with
the Data Protection Act 1998.
1. Storing and processing
information about me that I have supplied during this interview and
previously, or that I may later communicate to Holy Trinity PVT via your web site or otherwise, or that Holy Trinity PVT may
collect from third parties for the purpose of finding me employment in the UK.
2.
Transferring my personal data to and from Holy Trinity PVT
offices. I understand that Holy
Trinity PVT operates a computerised database which is accessible to Holy
Trinity PVT management and other
relevant personnel.
In order to process your application for temporary work, we may be required to disclose
all or some of the details
held in your personnel file, including but not limited to: your application
form; curriculum vitae; passport; birth certificate; driving license;
evidence of your right to work in the U.K; and
employment references, to actual
and prospective clients with whom you are,
have been or may be assigned to carry out work (including managed
service providers to whom clients
outsource the management of their temporary
recruitment.
In accordance with Clients’ processes
for safeguarding vulnerable adults and children, if you are placed into a role where you will be working unsupervised with vulnerable persons
we may also be required to disclose your copy of your Disclosure and Barring Service
(DBS) Disclosure to the Client. We may
scan a copy of your photo I.D. and where applicable your work visa and upload the scans to
the Client’s I.T. system.
We shall request that the Client complies
fully with its obligations under the Data Protection Act and all other relevant legislation pertaining to the safe
handling, use, storage, retention, processing and disposal of Personal
Data and/or Sensitive Personal Data contained with DBS Disclosures and that it:

does not disclose the DBS Certificate to any
third party and that it is passed only to those who are authorised to
receive it in the course of their duties except where disclosure is required by any applicable
laws, courts orders, or government regulations or where you give us permission
to do so

uses the information only for the specific
purpose for which it was required
·
does not keep Disclosure information for any
longer than is absolutely necessary
·
takes all reasonable steps to protect any
information submitted
You have a right to ask to see details of any personal information that a Client holds about you at any time. You may also request a copy of the Client’s policy statement on the Secure Storage, Handling, Use, Retention and Disposal of DBS Disclosures and Disclosure Information. During the
course of an assignment with a client you will have access to confidential information
and you undertake as follows:
1. In return for the client providing information to me in the course of my assignment as an Agency Worker with
the organisation, I promise to do the following in relation to the information
given to me or obtained by me in the course of such placement:
a. I promise to hold the information in the strictest confidence, and to
ensure that it is kept in a safe and secure place when not in use. I
acknowledge that no information is to
be removed from Client’s premises without the permission of the client.
b. I promise to use the
information only for the purpose of the work for which I have been given such information.
c. I promise not to disclose it
to any third party or to copy the information except as may be required
in the course of my duties.
2. I agree that any breach of
this undertaking by me or my third party to whom I release information may result
in legal proceedings being commenced against me including a claim for recovery
of any losses or damages incurred by the Client as a
result of that breach.
This document
contains information relating to the disclosure of your Personal Data as
defined by the Data Protection Act 1998(the “Act) to AF/AC, the Client you have been
assigned to undertake work for and the Audit Commission or
other bodies responsible for auditing or administering public funds. As such,
please read this document thoroughly before signing below.
The Audit Commission operates a National
Fraud Initiative (the “NFI”) in conjunction with audited bodies to identify fraud and overpayments relating to public money. The Client
you will be assigned to undertake work for participates in this scheme and by
signing this declaration you give your consent to your Personal Data being
released for the purposes of this anti-fraud initiative. Further details are

In accordance
with the NFI, Holy Trinity PVT will provide your Personal Data to the Client
who will in turn make this information available to the Audit
Commission and other bodies responsible for auditing or administering public
funds. All data will be provided in accordance with the Code of Data Matching
Practice which sets out the principles and practices adopted to
ensure your data is safeguarded and which has been sanctioned by the
Information Commissioner.
You have a
right to ask to see details of any personal information that Holy Trinity PVT
holds about you at any time. You may also request a copy of Holy Trinity PVT policy statement on the Secure Storage, Handling, Use, Retention
and Disposal of DBS Disclosures and Disclosure of Information.
In signing
this registration form you confirm that have read and understood the
information detailed above and that you give your express consent (a) for
you to disclose the entire contents of your personnel file to employees
or agents of Holy Trinity PVT or the client; (b) for Holy Trinity PVT or the
client to verify any of the details disclosed; and (c) for the client to release
your Personal Data to the Audit Commission and other bodies responsible for
auditing or administering public funds.
